According to art. 13 of the General Regulation on the Protection of Personal Data of 27 April 2016 (Journal of Laws UE L 119 of 04.05.2016), I inform that:

o entities involved in the implementation of the order
- Mr. / Mrs. personal data will be stored for 5 years / or the data will be processed until the processing stops for business planning purposes);
- You have the right to request the administrator to access personal data, rectify them, delete or limit processing;
- You have, Mr. / Mrs. the right to file a complaint with the supervisory authority
- Providing personal data is voluntary, however refusal to provide data may result in refusal to conclude the contract;
